8. Declaration and Signature: I have read and understood the terms of this consent form. The intended purpose is not against my/my ward (in case the guardian is giving consent) moral and material interests. Therefore, signing below, without any coercion and by free will, I acknowledge my consent to the processing of my personal data as outlined above.
